Would really appreciate a heads up on which cell phone carriers use SIM cards in the US and specifically which provide SIM only i.e. pre pay so can insert the SIM card in any cell phone that is unlocked. I know T-Mobile do (or did a year or so ago when I was working in US)

I think AT&T and Sprint at the only other ones that uses SIM cards.

I'm not sure why Sprint would as it's a CDMA network, right?

I thought Nextel phones had sim cards. I could be wrong though.

I'm pretty sure Sprint/Nextel use sim cards, but they are on a CDMA network, (don't hold me 100% to that, cause it's been a while since I've used those services. I'm positive that T-Mobile and Cingular/ATT use sim cards on a GSM network, and both of them have pay-as-you-go plans.

To confirm, in the USA, AT&T/Cingular and T-Mobile are GSM providers that use SIM cards.
Sprint PCS phones are CDMA and do not use SIM cards, however Nextel (3-Digit Series) specific phones do use SIM cards but don't work with the SIM cards found in AT&T or T-Mobile phones because of not being a GSM provider.
