I seem to have excellent luck with the following:
BadBehavior - Main defense
Akismet - Handles the spam directly
Tam Tam's Simple Spam Filter - Clears out the obvious spam from Akismet

Why BadBehavior is Main defense?

BadBehavior catches at least 50% of the incoming spam attempts. It blocks them from even making a comment, so Akismet doesn't have to filter those spam comments.
You obviously would get nowhere with just BadBehavior, but if you have a second line of defense, BadBehavior does an excellent job controlling the spam problem.

I'm glad this note popped up. I don't know what's up, but lately, Akismet has been letting quite a few blatant spam comments through its defenses, into my moderation queue. When you look at them, filled with viagra links and other such junk, you would think that Akismet would catch them, just like the other thousands of comments its caught with viagra, etc. in it. I'm not sure what's up.
At any rate, I'm going to get Bad Behavior a try. Looking at the site, it sounds like a pretty good solution.

I don't understand–when spam is blocked by spam management plug-in, then why spammers are spaming further...?

Good question. I still don't understand the business of sending out massive spam like that? Is it even a business? Or it's just some black hat effect?
I use a combination of Bad Behavior and Akismet, but recently some spam has been getting through both those defences as well. The development of Bad Behavior has hit a snag though because it hasn't been updated in a long long time. Still, as long as I don't resort to using Captcha's, I can handle the occasional incoming spam.

I am using Akismet, Working better for me.. I have registered for a Defensio but never used it :) 'coz of Akismet ;)

Akismet works just fine for me. Once a day, I refresh my queue and most of what gets into my moderation list gets cleaned out.

WP-SpamFree with Akismet gives good result...I don't have found any comment spam from the date I have implemented it (WP-SpamFree) in my blog Espreson

Been using Wordpress for over a year, and Askimet's doin all the work!
I have an heavy traffic on my site, and legit comments got tagged only few times... should have a look on it the first weeks/months

Bad Behavior + Akismet + Referrer Bouncer.
I may be a unique case but I seem to have more trouble with bogus referrers than with comment spam.

A preview button before the spammer can even post a comment.
